چکیده

A 68-year-old male patient was referred because of an irregular heart rhythm. The electrocardiogram (Fig. 1) showed sinus bradycardia with interpolated premature ventricular complexes (PVCs) occurring in a trigeminal pattern. There are two interesting observations in Figure 1. First, there is an alternating pattern of P-P intervals, with intervals of 1,320–1,340 ms whenever there is a PVC between two conducted sinus beats and longer P-P intervals of 1,420–1,470 ms when there is not. Secondly, there is also an alternating pattern of P-R intervals, with a P-R interval of 280 ms fol-

Premature Systoles Originating in the Sino-auricular Node.

Premature systoles arising in the sino-auricular node are characterized in the electrocardiogram by premature P waves that are identical in contour with other sinus P waves and by the absence of a prolongation of the returning cycle. Actually the returning cycle may be shorter than the normal cycle because of prolonged sino-auricular conduction of the premature beat (Wenckebach, 1908). Accordin...

Treatment of frequent ventricular arrhythmia with encainide: assessment using serial ambulatory electrocardiograms, intracardiac electrophysiologic studies, treadmill exercise tests, and radionuclide cineangiographic studies.

The effects of encainide on ventricular arrhythmia and left ventricular function were studied in 21 patients with chronic, high-grade ventricular arrhythmia using a prospective, 3-month, placebo-controlled, single-blind trial design. Encainide caused a 96% decrease in the average hourly frequency of ventricular premature complexes (VPCs) and comparable reductions in salvos of nonsustained ventr...

Images and Case Reports in Arrhythmia and Electrophysiology Premature Beats and Unexpected Heart Block An Unusual Mechanism Confirmed by Ablation

A 23-year-old, otherwise healthy woman presented with recurrent skipped and rapid sustained heart beats. Her physical examination and echocardiogram were normal. The presenting ECG in Figure 1 shows sinus rhythm, a nonconducted sinus p wave, and premature complexes. Both left bundle-branch block (5 and 6 complexes) and right bundlebranch block (10 complex) morphologies are present. The 5 and 10...

QT interval parameters and ventricular arrhythmic events in liver cirrhosis- correlation with severity and etiology

Electrophysiological abnormalities in cirrhosis, such as the prolongation of the QT interval, are associated with higher risk of ventricular arrhythmias. Holter monitoring offers a full picture of these during 24 hour. Our study aimed to evaluate the extent of the QT interval prolongation, identify etiological and biochemical elements linked with it, and investigate the correlation with ventric...

Paediatric chest wall trauma causing delayed presentation of ventricular arrhythmia

This report describes a paediatric patient presenting with haemodynamically stable non-sustained ventricular tachycardia 1 day after minor blunt chest trauma. Initial laboratory studies, chest X-ray and echocardiography were normal; however, cardiac MRI revealed precordial haematoma, myocardial contusion and small pericardial effusion. Throughout her hospital course, she remained asymptomatic a...
